John Patience Mackellar was born on 15 April 2351, in Temtibi City, on Risa. His parents are Hal Mackellar, a linguistics professor, and Dorothy Greenway, a law librarian. The family was living on Risa at the time of John's birth due to his father's teaching position at the University of Risa, but his father's family comes from Mars Colony and his mother's from New Melbourne Colony.  He has a sister, Donna, and a brother, Mitchell, who are three and eight years younger than him, respectively.  When John was eleven, his parents divorced, and he moved with his mother and siblings to New Melbourne, where he lived for the remainder of his childhood.  The young John Mackellar found living on New Melbourne, a predominantly rural colony with a sparse population and temperamental climate, hard to get used to after spending his entire life up to that point on the climate-controlled, tourist-oriented Risa.  It was the isolated and unpredictable nature of New Melbourne that first enkindled John's interest in medicine.  As a teenager, he was a member of the colony's first responder team.

After finishing secondary school on New Melbourne in 2369, John moved to Betazed, where he worked as a medical technician at a hospital in Medara for two years.  In 2371, he was accepted into the College of Medicine at Enaren University, and studied there for three years until the Dominion invasion and subsequent occupation interrupted his studies. John immediately attempted to join one of the resistance cells forming in the wake of the occupation, but was rejected because he was not Betazoid.  Despite never being a full-fledged member of the resistance movement on Betazed, he used his skills as a medic and survivalist to aid them tangentially whenever possible.

John resumed his studies at Enaren University in 2376, shortly after the occupation ended.  The term before his graduation, he completed a summer internship at a clinic in the village of Jarkana.  After graduation, he decided to pursue a full medical degree, and was accepted into the University of Betazed in 2378. During med school, he was captain of the university's debate team, and placed second at an inter-university competition on Vulcan in 2380.  Specializing in survival medicine, including desert, arctic and water survival, John earned his M.D. from the University of Betazed in 2381, and immediately found work at a hospital complex in Rixx, one of Betazed's major cities.

By 2384, Betazed was well on the way to recovery from the occupation, and John sought a change in position that would allow him to utilize his specialties in a new way.  He spent a year on Cardassia Prime, doing humanitarian work in medical facilities all over the planet.  His poor knowledge of Cardassian physiology initially limited his effectiveness as a physician.  He spent another year working with Federation and independent relief organizations following the destruction of Romulus in 2387. By 2389, he had returned to Betazed and was working as an attending physician in a teaching hospital in Medara.
