Pt. seems to be adjusting well to the new medication (Sertraline, now 150mg). Pt. is not experiencing any side effects and appears to be adjusting fairly well to the prescribed dosage. Recommended pt. continue his attempts to "self-evaluate" how he is feeling at specific points (mindfulness). Recommended pt. continue medication (Sertraline, now 150mg) with regular visits to Dr. Gates (psychiatrist). Pt. to continue counseling.

Chythar is optimistic and brings his therapy dog, Devlin, to assist with the session. Setting for the session is the holodeck, a beach program he appears to have designed himself. Pt. expresses minor issues with the new medication (Sertraline, 50mg), stating that he has difficulty remembering to take them. Pt. is experiencing minor anxiety, a normal side effect for this particular medication. Pt. is not experiencing any other side effects and appears to be adjusting fairly well to the prescribed dosage. Recommended pt. seek feedback from his colleagues regarding changes in mood and that he attempt to "self-evaluate" how he is feeling at specific points (mindfulness). Attempted an Emotion Exchange Meditation. Pt. was receptive, if somewhat anxious. He demonstrates excellent control over his thoughts and emotions. The Giving and Receiving goes well, though pt. notes some disorientation and feelings of being overwhelmed at the close of the Exchange. Pt. seems receptive to further Exchanges. Recommended pt. continue medication (Sertraline, 50mg) with regular visits to Dr. Gates. Pt. to continue counseling.

Chythar Skyfire gives an initial impression of a model Starfleet officer. His appearance is clean and his posture is confident. He responds appropriately to social gestures and cues, despite a previous diagnosis of mild autism. His mood is optimistic, though it is clear he harbors traumas in his past. He shows signs of depression and anxiety, understandable given what he has been through over the last few months (year). He demonstrates a strong command of his emotions and is able to acknowledge those feelings without allowing them to control him. Memory and cognition are good overall with minor long-term memory difficulties, perhaps due to low-grade concussions from the multiple explosions he has been caught in. During the consult, he mentioned a developing romantic relationship that seems to have had a positive effect on his disposition and mood. Pt. notes that he is not taking any medications regularly at this time. Pt. reports previous difficulties with hallucinations and hearing voices as well as previous abuse of neural suppressants and painkillers, though he states that these past issues do not impact his ability to perform his duties. Pt. expresses no suicidal thoughts or desires and has no thoughts of harming himself or others. Pt. states that his sleep and eating patterns have improved since visiting family. He has a strong support system in both his personal and professional lives. An alternate location for sessions would be beneficial in that it would allow the pt. to feel more in control of his surroundings. Recommend continued counseling as well as pt. begin Sertraline (Zoloft) 50mg for depression, anxiety and autism issues. Referred to Dr. Isaac Gates, Psychiatrist, for medication management.

  • Medical Notes: Chythar Skyfire is a highly capable medical officer showing vast skill in research, data correlation and creative solutions to complex problems. He has shown to do good work under pressure, though expresses much frustration when working under non-ideal conditions.

    Dr. Skyfire exhibits a range of limitations classified on the mild autism spectrum, notably including a lack of demonstrative empathy for others, and difficulty in communication. However, the inability to demonstrate empathy does not equate a lack of empathy for others. Through various counseling techniques, Dr. Skyfire has been working on overcoming these limitations. Unfortunately this has been stalled somewhat by a very chaotic transfer situation where many crewmates frequently are moved away from Dr. Skyfire, initiating a lack of trust in both environment and others. I believe that a stable situation with a clear chain of command will be the best environment to bring out his talents and allow him to overcome his limitations.

    Chythar Skyfire also shows an atypical communication structure when under intense stress, which may include switching dialects or languages from a broad Terran base. There is nothing to show any brain damage in connection with the phenomenon, and it should not be taken as aphasia. It is more likely an unusual stress-reaction connected to emotional withdrawal. Recommended course of action is to remove the cause of stress from the immediate vicinity, and allow Dr. Skyfire to regain control of himself and the situation.

  • Medical Notes: Ensign Skyfire has the appearance of an individual who is in control of who he is and what he is about. Seemingly a self proclaimed "workaholic" he has professed to be uncomfortable and stressed when work cuts into his free time. This provides a conundrum: does he really throw himself into his work and prefer it above other activities, or does he say that because he feels he should to impress those around him?
  • Has expressed desire to separate professional from personal, which is impossible (in the opinion of this humble counselor of course.)
  • Exhibits tendencies for self defeating behavior which manifests as frustration. This frustration lashes out and nearby sensitives such as empaths, telepaths, and those sensitive to such energies feel the brunt of it. Has expressed a desire to control his emotions for this reason.

NOTE: Feels he has gotten on Dr. MacLaren's bad side. Will keep an eye on the situation for any developments.
