PHYSICIANS' NOTES

  • SD 239405.01 - Medical Officer: Cmdr. Garner, Starfleet Academy
Medical Assessment: Cadet Knight is a 23 year old human male. He has a lean physique and at present is within normal weight for his age. I have advised Cadet Knight to continue his present diet. He engages in physical activity on average five times a week, the majority of which are of a semi-intense nature. He advised he does not take any substances and has no history of substance absence. He does not partake in alcohol regularly and drinks little to none. He has no medical history of note and no family history of major illnesses. I have conducted the standard Starfleet medical exam and have found Cadet Knight to be a fit and healthy individual and confirm at the present time he is fit for graduation and assignment within Starfleet.
  • SD 239405.01 - Medical Officer: Cmdr. Cattan, Starfleet U.S.S Atlantis
Medical Assessment: Lieutenant junior grade Knight has completed surgery to have an Ocular implant surgically added to replace his non functional left eye. Injury was sustained on an away mission and steps were taken to save the eye but damage was too extensive. Implant is functioning well and I do not fore see future problems. I recommend monthly check ups for at least a year, no physical or physiological problems are apparent at this time. Recommend initial counseling sessions.
