  • Incident: During the 2367 Borg incursion of the solar system, Master Chief Petty Officer Vada failed to man her post aboard Earth Spacedock.

  • Disciplinary Action Taken: None taken (See remarks)
  • Disciplining Officer Remarks: Given the extenuating circumstances of Ms. Vada’s previous traumatic encounters with the Borg, service with multiple crews killed immediately prior at the Battle of Wolf 359, and current plans to retire, the inquiry has determined that Ms. Vada’s failure to report was the result of an episode of post-traumatic stress and not any willful failure or incapacity on her part. The incident has been noted on her disciplinary record, but no further action was deemed necessary or taken.
