Mal'Aari Syndrome is a rare genetic neurological disease which can affect one in 1 million Vulcans. The disease is not restricted to specific areas of the Vulcan brain, and so there are numerous symptoms which mirror other diseases, causing a proper diagnosis to be very difficult. Mal'Aari Syndrome can affect those as young as one year old, and is a life-long illness, as the only known cure is gene resequencing therapy, which is currently outlawed within the United Federation of Planets. Treatments need to be tailored to the specific symptoms of the disease, and need to be administered according to a schedule designed by the patient's attending physician. Failure to do so could result in immediate deterioration of the patient's health, and possibly death.

T'iana, the daughter of Starfleet officer Varaan and his wife T'lani, has Mal'Aari Syndrome. She requires weekly treatments to forestall the degenerative neurological effects on the areas of her brain responsible for controlling her emotional response (similar to Bendii Syndrome in the Vulcan elderly), and the areas responsible for some cognitive processing, specifically in regards to social interactions.
