You can use this template to create a First Contact report from your character's point of view: Create a new page titled "Species Name/First Contact" Insert {{subst:First Contact}} into the body of the new page and click save. Click edit and fill out the details as you wish. The Reporting officer's reference number is from the StarFleet Serial Number Register and is also used as the student ID in the academic transcript template from Starfleet Academy.
