For this same template, but without the ship at the top, use Template:CharSum. To use as the intro blurb on your character page (or for any other reason), just type the following:

Example

USS Endeavour NCC-1895-A Galaxy class Commanding Officer: Geoffrey Macarius First Officer Henry Jackal Henry Jackal: Past to Present Prisoner, First Officer Having been captured on the Endeavour's first mission and placed in the brig, Jackal was released and asked to assist Captain Macarius in understanding the disaster and getting the ship back to its own time after the entire crew, save the captain and himself, were killed by an unknown spacial and temporal anomaly.
