Subject: Self-Defense Class Injury Incident

Personnel Involved: Lieutenant Junior Grade Aine Sherlock, Lieutenant Junior Grade Meidra Sirin, Crewman Second Class Hamsan Dwich

Incident in Question: While conducting an unarmed self-defense class in which I had asked Counselor Sirin to demonstrate Vulcan martial arts techniques, Crewman Hamsan's arm was fractured while Counselor Sirin was demonstrating an armlock. Similar injuries have occured in similar classes and is an existing risk with classes such as these, in particular when multiple species are involved. As the OIC (Officer In Charge) of the class, I take full responsibility for the injury sustained by Crewman Hamsan.
