COUNSELOR REPORT: SD 239701.23 Condition: Follow-Up Counselor: Counselor Notes: Patient requested telepathic and empathic re-evaluation following a procedure on Vulcan. The requested re-evaluation revealed patient has reduced telepathic and empathic abilities. Former rating was T2/E4. Recommend patient continue weekly counseling and use of neural suppressants as needed. Also, recommend the patient discontinue the use of Sertraline as the patient no longer reports depression symptoms. Continue use of Zoloft as needed for treatment of anxiety.--QT Medical Notes: Per counselor recommendation, the patient's dosage of neural suppressants has been adjusted to dosage noted in medical records.
