After returning to the present from the year 1943, the Tiger crew was assaulted by torpedo fire from the Klingon vessel, the IKS Na ra'den.  Commanded by D'Ghor, they were on a mission to establish a colony on Bilire IV, crushing the Romulans that lived there and establishing their own presence in the Ithassa Region.  The Tiger was set to pursue, but found that their engines had been damaged in the blast.  After a couple of days, warp engines were back online at full capacity where the Tiger and her crew went on a mission of mercy to Bilire IV.
Upon arriving at the planet, the crew found themselves in about as much misery as the Romulans down below.  The Klingons were no where to be found and there was much hostility towards the Federation.  Colonies were ravaged by the "Wrath," a disease which displayed symptoms of both Tuberculosis and Staph.  Needless to say, investigative parties sent to the service were met with weapons fire and disdain, resulting in expulsion from the hospital down below as well as armed combat.  Finding themselves outnumbered and out gunned, the away team requested reinforcements leaving the Tiger with a skeleton crew.
Meanwhile, the Klingons lay in wait while the Federation tried unsuccessfully to assist the populations.  Moving by stealth through the caverns underneath Bilire IV, they planned intricate attack formations that would have been enacted had not yet another major player appeared in the region.  The night before the assault, D'Ghor was attacked by one of the non-coporeal entities, the Reapers, specifically Dodhun "Third of the Father."  This would have been the end of the invasion, but Kon'Jah, the former First Officer, took command of the troops while staking his life on finding who had killed his Captain.
The Klingons, temporarily delayed, gave time for the reinforcements to make some progress.  Beaming down to the hospital, there was a desperate attempt to find a cure to the Wrath while the Captain and her team tried to diplomatically resolve their dilemma.  Unfortunately, things went from bad to worse as an intense weather pattern assaulted the planet rupturing the dams towards the main colony and hospital.  Hundreds to thousands of colonists lost their lives due to the flood and the inability to raise the shields at the hospital on time.  Making matters worse, several of the Tiger crew were then kidnapped by the Reapers and brought upon their vessel, halting the development of a cure.
Trying to regroup after the flood, groups were ambushed by Kon'Jah and his team.  Luckily, after one and one combat, the Captain resolved not to molest the Starfleet officers any more after losing honourably.  Not all Klingons felt the same as a splinter group led by Tanas decided to wreck havoc until the Reapers destroyed their vessel, stranding them on the planet.  Seeing the opportunity before them, the Tiger crew left the planet in pursuit, but not before a bungled order to bring the crew aboard also resulted in the transportation of the colonists to the ship as well.
Soon following was a break of quarantine with the Wrath resulting in a shipwide epidemic.  In combination with the losing attack on the Reaper vessel, all hope looked to be lost for the crew.  However, with the resourcefulness of the Engineering department and Tactical, new weaponry was developed and the crew managed to pierce shielding and gain access.  While the rescue mission was being mounted, Medical made a breakthrough discovery and a cure was issued immediately.  Soon after, the away team returned and the vessel was summarily destroyed.  Cure in hand and the Klingons bested, the people of Bilire IV were returned to their planet.
