INCIDENT REPORT: SD 229601.01 Filed Charge: Intoxicated on duty, Dereliction of duty Incident: Several crewmen attending a New Year’s Eve party were absent from duty the next morning due to intoxication. Disciplining Officer: Captain Crorash Disciplinary Action Taken: Note added to disciplinary file, assigned to extra maintenance details Disciplining Officer Remarks: As ordered by Captain Crorash for all attendees of Crewman Reily’s New Year’s Eve Party.

INCIDENT REPORT: SD 229804.12 Filed Charge: Tardiness Incident: Crewman Vada was chronically tardy, late to reporting to duty posts more than five times in a single week. Disciplining Officer: Ensign Donna Swift Disciplinary Action Taken: Note added to disciplinary file, additional duty shifts ordered for one week Disciplining Officer Remarks: Due to recent events, a full letter of reprimand was not considered warranted, but I have firmly informed Vada that this should not become a repeated behavior.

INCIDENT REPORT: SD 230604.14 Filed Charge: Insubordination Incident: Petty Officer Vada was insubordinate to a lieutenant from engineering, publicly questioning orders and denigrating the lieutenant and his character. INCIDENT REPORT: SD 236702.26 Filed Charge: Dereliction of duty Incident: During the 2367 Borg incursion of the solar system, Master Chief Petty Officer Vada failed to man her post aboard Earth Spacedock. Disciplining Officer: Rear Admiral Mallory McClain Disciplinary Action Taken: None taken (See remarks) Disciplining Officer Remarks: Given the extenuating circumstances of Ms. Vada’s previous traumatic encounters with the Borg, service with multiple crews killed immediately prior at the Battle of Wolf 359, and current plans to retire, the inquiry has determined that Ms. Vada’s failure to report was the result of an episode of post-traumatic stress and not any willful failure or incapacity on her part. The incident has been noted on her disciplinary record, but no further action was deemed necessary or taken.
