PHYSICIANS' NOTES

  • 239208.10 - Medical Officer: Dr. Soren Hapstadt, MD, Starfleet Medical
  • Medical Assessment: Regan has demonstrated considerable strength of will to overcome his serious alcohol dependency and withdrawal symptoms. I have started him on a course of Acamprosate to ease the withdrawal, but I'm hesitant to prescribe anything more. He seems determined to work through the issue with the aid of modern counselling, and medically that's all I can prescribe at the moment. Physically he is in good shape, and the wounds he suffered in his absence from the Academy have been treated successfully with dermal regenerators and other traditional methods.
  • 239708.17 - Starfleet Chief of Staff Vice Admiral Dashaya Ashanti, Chief of Staff, Starfleet Command
  • Addendum: By order of Starfleet Command the following caveat is to be placed on permanent file for Officer JM - 058 - 399: A mandatory substance test must be carried out by current Chief Medical Officer (or any qualified Medical Officer) upon arrival at current posting, be it any Starfleet vessel or installation, and the results placed in permanent file. Further testing is authorized by any officer (of any rank) in the Medical Department, be it with or without the agreement or consent of current Commanding Officer, if he/she/they has any evidence or suspicion of relapse or continuation of dependency on controlled substances while on active service in Starfleet Command. For any willful, anomalous, or intended breach or failure of said test, the outcome will be effective immediate dismissal of post and forfeiture of commission within Starfleet Command. Signed, Vice Admiral Ashanti, Starfleet Chief of Staff
  • SD 239712.17 - Medical Officer: Fenro, MD, Caldonian Vessel Syrtan.
  • Medical Assessment: Lieutenant Wilde underwent facial surgery after an incident which saw his right eye damaged by a pointed blade. Steps were taken to remove the eyeball and repair the tissues to his eye socket in order for a synthetic replacement to be installed. We constructed the synthetic in our labs on the Syrtan after taking a genetic scan of the patient. Installation of the new synthetic eyeball took place on 239712.17 on the Syrtan and the patient is recovering well.

COUNSELORS' NOTES

  • SD 239309.01 - Deputy Chief Counselor: Dr Simon McIntosh, Ph.D, Psy.D, Starfleet Medical
  • Psychological Assessment: I have been working with Regan over the last few months in regards to his recovery and insistence on returning to Starfleet Academy. He is a very strong willed and determined young man, and well liked by Starfleet Medical counselling staff, but behind his bravado and openness lies a very fragile and sensitive man. He has shown a tendency to mask his true emotions or feelings with jokes and sarcasm, which are usually transparent cries for help. His medical recovery has been strong and on track with no deviations or relapses and I believe he has come through his past ordeals strongly. He is passionate about resuming his studies and I am pleased to see his determination in returning to Starfleet. I believe he has come to terms with the degeneration of his studies over the last few years, but I am recommending mandatory counselling during his return to the academy until such time he graduates and is posted to a position within the service, to help him keep focus and work through any issue that should occur with him going back to school.
