PHYSICIANS' NOTES

  • SD 239110.18:
Medical Officer: Lieutenant Talia Kaji, MD, USS Victory
Addendum: Treated for wounds received across the shoulder, chest and arm. The people aboard the Lady Adventure are primitive pirates who wouldn't know modern medicine if it bit them in their peg-legs. Also repaired a patch of skin on the back of his right hand for shoddy IV work. If he begins to feel dizzy or nauseated, he is to report back to sickbay immediately.
  • SD 239201.07:
Medical Officer: Talia Kaji, MD, USS Victory
Addendum: Repaired torn eardrum, but pressure balance may be off. Will need to watch for vertigo symptoms in case of BPPV.
  • SD 239203.02
Medical Officer: Ensign Ilyazi Malon, MD USS Darwin-A
Addendum: E.A.R.S. surgery successful to left ear.
  • SD 239207.11
Medical Officer Lt. Commander Solok, MD Starbase 118 Ops
Addendum: E.A.R.S. surgery successful to right ear.
  • SD 239210.10
Medical Officers Lieutenant JG Benjamin Ford MD, Ensign Shakkar Jakobi
Addendum: Patient suffered grade 3 concussion, minor spinal trauma.
Medical Officer Dr. Ryan Lanford, MD
Addendum: Clean bill of health. E.A.R.S. units fine and still fully functional.
  • SD 239712.25:
Medical Officer: Lieutenant Karise Indobri, MD
Addendum: Patient sustained multiple hairline fractures to the left wrist. Treated with anabolic protoplaser and ordered to wear a wrist brace as a precaution in addition to light duty for a few days.

COUNSELORS' NOTES

  • SD 239204.24:
Chief Counselor: Lieutenant Vance Sheridan, Starbase 118 Ops
Addendum: On being asked how the last few days had been, Lieutenant Peters described his anxiety over the frequency of his transfers over the last few months. It seems he has had little time to settle down before moving on in each case. Recommend that he remain assigned to Starbase 118 for at least a period of six months to allow him to adopt a more normal routine and settle down.
His description of his experiences aboard the USS Victory and USS Darwin indicate potential awkwardness in getting to know people. The lieutenant referred to a mission on the Darwin where he fell victim of an anomaly that prevented effective communication with the rest of the crew, and to a rejection aboard the USS Victory. Advised that he should make an effort to connect with his current crewmates and suggested Lieutenant (J.G.) Rix, a departmental colleague, as a good example.
I am happy to declare Lieutenant (J.G.) Peters fit for duty.
  • SD 239603.03:
Counselor: Lieutenant Corix, Utopia Planetia Fleetyards
Addendum: Over the course of three years, Cadfael's anger difficulties have been channeled into his studies as he majored in tactical. He now has a skill set that has taught him how to interact with people on a level that has turned him into a valuable member of Starfleet society. He is a very tactical-minded person, and the fact that he has majored in it recently has granted him additional clarity.
  • SD 239707.14:
Counselor: Ensign Airik Tierney, USS Juneau
Addendum: Tierney: =/\= Counselor’s Log, current stardate. Lieutenant Commander Cadfael Peters counseling session was a standard initial crew evaluation. He seems well adjusted with strong coping strategies. Peters is cognizant of his relationships and interested in building them with his team. His interest in a crew injured in his department, though he was not stationed on the Juneau at the time, is a good sign of his leadership ability. Lieutenant Commander Peters does not need regular counseling and is fit for duty at this time. Computer, end log and sign Airik Tierney, MD, ship’s counselor. =/\=
  • SD 239710.08:
Counselor: Ensign Airik Tierney, USS Juneau
Addendum: Lieutenant Commander Cadfael Peters stopped in for a counseling session to process a situation that occurred. During this situation he misfired at Commander Orrey. While a genuine mistake, Peters is holding some guilt because of it. I've suggested he process the event and think about ways it could go differently in the future. Computer, end log and sign Airik Tierney, MD, ship’s counselor.
