  • Purpose of Visit: Follow-up (Post-Surgery)
  • Medical Assessment: Lieutenant Hyden was seen for a follow up regarding the functioning of her implanted cardioverter-defibrillator. Patient reports no further instances or symptoms. Patient has been instructed to report to Sickbay immediately if symptoms return or any new symptoms arise.

  • Purpose of Visit: Potential syncope
  • Medical Assessment: Lieutenant Hyden was brought to Sickbay following a syncope episode. As this isn't the first incident, the attending physician (myself) recommended surgical implantation of a cardioverter-defibrillator to control heartrate and prevent further instances of Syncope. Surgery is to take place tomorrow.
  • SD 239511.30 - Medical Officer: Wyla Avae, Nurse, USS Atlantis
  • Medical Assessment: Ensign Hyden was beamed to Sickbay in an Emergency Transport. The Ensigns blood pressure was lower than it should have been. An elevated heart rate and her breathing was rapid. The diagnoses I deduced was Syncope. I told the Ensign to check in if she had any more issues.
