Medical Assessment: Cadet Thoran is a 23 year old human male. He has a lean physique and at present is borderline underweight. I have advised Cadet Thoran to eat a few good meals to increase his weight. He engages in physical activity on average four times a week, the majority of which are of an intense nature. He advised he does not take any substances and has no history of substance absence. He enjoys a glass of whiskey on average three times a week. He has no medical history of note and no family history of major illnesses. I have conducted the standard Starfleet medical exam and have found Cadet Thoran to be a fit and healthy individual and confirm at the present time he is fit for graduation and assignment within Starfleet.

Counselling Assesment: Overall, Mr. Thoran appears to display many of the personality traits and behavioral characteristics often seen in people who succeed as Starfleet Officers. This cadet’s cognitive abilities and interest patterns are consistent with those seen in people who typically do well in this profession. Results of this assessment suggest that Mr. Thoran has no emotional, behavioral, or psychiatric impairment that would prevent him from fulfilling the responsibilities of the Security or Tactical positions within Starfleet. Given these findings, it is strongly recommended that he be approved for graduation and assignment.
