Ben Edwards was admitted to Starfleet Medical Facility 5347 on stardate 239012.26. He was caught in an explosion outside starfleet marine headquarters, he has sustained damage to both eyes from the intense flash and a cut to the back of the skull. The eyes and head wound healed. While under testing he was found to have a low Vitamin A level, and has been placed on supplements to be taken once a day for 30 days. Note placed in medical file for retesting on stardate 239101.26. Until vitamin A levels return to normal I've recommend his removal from flight status. Attached files below.

Ben was restricted by DR Todd Manius from carrying a weapon due to his problems with his eyesight and from going on away missions.

Ben Edwards has been given the all clear medical to return to full marine duties and able to fly shuttle, Fighters again. He still has flash backs of the explosion that cause the damage to his eye.

Major Ben Edwards is cleared by the medical department for the upcoming mission. Unfortunately, he will have to wait to see Counselor Reil for his psychological assessments. The mission trip had top priority, as he was called up for duty in the away party. This was after the physical was completed. I do have a few reservations, all minor, that will not interfere with his ability to work.

1) Major Edwards has scar tissue in his occidental regions from an old injury. What concerns me a nerve is entwining itself directly into the area between the scar tissue, and brain matter. Eventually it may begin to cause severe headaches.

2) The Majors creatinine levels are a bit high suggesting he may be developing a urine infection. He also suffered from a lack of vitamin A, for which he received an hypo-spray injection of 0.5 units of vitamin A, He is to come back after the mission. If his creatinine levels do not decrease by itself, I will give him an injection of 10mg Lovastatin, if the creatine levels do not decrease. Same with his Vitamin A levels. I would give him another injection, but I am betting after this injection he received today, there will be know more problems. Also his white blood cells were also elevated, again pointing to a possible infection.

3) Since his last eye exam Major Edwards eyesight is beginning to change slightly. I did see some scar tissue in the back of the (L) eye as a result of an old injury. His vision in the (R) eye is 20/15. The vision in the (L) eye was 20/20. He complained of momentary blurriness, but it cleared up right away. All this means is I will check the eye again in 6 months to a year.

4) my last concern was the Major evaded any question about his family history. He may have a reason for it, or he may have some psychological problem talking about his family. I sent a note to Counselor Reil, and I will leave the issue to the counselor. As far as the medical dept. is concerned, the major has past his physical,and is fit for duty aboard the USS Gemini, as there were no problems when scanned both by the medical tricorder, and the OBC scan.
