PAST HOSPITALIZATIONS/SERIOUS ILLNESS

  • 239002.17: The day of his cadet cruise, the patient broke his left wrist. Bed rest was prescribed, and the patient decided to use a dermal regenerator to aid in the healing process. Wrist is subject to irritation once in a while.
  • 239010.11: The patient suffered a low grade concussion and doesn't remember what happened before or after the blast. Did not lose consciousness, suffered two ruptured ear drums. Both of which were treated by Dr. Folds. Bed rest was prescribed by both Captain Nicholotti and Dr. Morgan, while the patient healed most of his own injuries using a dermal regenerator.
  • 239101.19: Patient sustained a second break to his left wrist. Attempted to fix it himself and may still have some residual nerve damage in his fingertips.
  • 239103.18: Patient was sedated by Dr. Alexander Morgan following intense pain throughout his entire body from a Dokkoran mind-meld. Said experience altered Chythar's genetic code such that he became somewhat telepathic/empathic, though exact status is unknown at this time. Suspected to be T2/E2.
  • 239110.17 - 239110.25: Upon discovering a connection to a previously used strain of the Skyfire Virus, which was regulated to Romulans, he employs similar methodology by injecting himself with a different strain that was geared toward Cardassians. He did this to himself on purpose, and the resulting tissue degeneration occurred in his left wrist. The antigen came through, and his experiment ended. Performed under medical supervision at DSX. Telepathic/Empathic Status changed to T4/E6. See Psychological Profile for full notes.
  • 239110.28: Patient sustained a broken rib after getting thrown into a wall by a shockwave caused by an explosion on DSX. Treated by DSX nursing staff.
  • 239211.13: Cracks a rib during a fight with a targ/wolf thing on the holodeck.
  • 239212.17: Treated for rib injury.
  • 239209.20: Treated for a black eye. Patient also expressed concern over loss of telepathic sense. Tests are being scheduled to assess the damage.
  • 239304.13: Fractured hand in multiple places by punching a table.
  • 239306.02: Ruptured left eardrum in an explosion.
  • 239306.23: Ruptured both eardrums in a pressure explosion, cracked rib from impact.
  • 239311.17: Sprained left wrist.
  • 239311.30: Received a black eye, right side.
  • 239312.05: Ruptured right eardrum in another explosion.
  • 239312.26: Treated for injuries sustained on 12.05. Cumulative damage to both ears required surgery and EARS implants.
  • 239401.04: Sprained right wrist while snowboarding.
  • 239406.30: Initial exposure to a lethal virus during first contact with a race known as the Hocktin, caused by a flea bite; first symptoms appeared on 239407.02 and included partial aphasia, tremors, fever, diminished focus, headache. Vaccine was administered on 239407.06.
  • 23940907: Sprains both wrists in a fall.
  • 239411.30: EVA suit failure causing momentary loss of consciousness. Emergency beam-out to sickbay required. Patient rendered temporarily deaf.
  • 239502.08: Third degree phaser burn on right leg epidermis. slight vaporization of surrounding fatty tissue. Concomitant contusions extending throughout the lower leg.
  • 239510.13: Treated for minor bruising to the ribs. Complained of light headache. EARS device reinstalled.

PAST SURGICAL HISTORY

  • 239111.23: Full corrective wrist surgery to left wrist. Instructed to keep a brace on it until further notice.
